Center Policies

Clinic Policy

Dr. O.K. Medical Center maintains a standard set of policies designed to maintain optimally efficient relationships with all our patients. These policies are necessary in order to provide responsible and timely service to all patients.

Please be aware that Dr. O.K. Medical Center is a teaching facility, and you may be exposed to Medical Students or Residents.

Information/Record Keeping Policy

Dr. O.K. Medical Center requires a certain amount of personal information in order to properly service patients. The information provided to Dr. O.K. Medical Center is kept strictly confidential and secure. Dr. O.K. Medical Center is responsible for the privacy of all patient data and as such:

  1. No patient medical or health information may be given over the phone. This includes laboratory results.
  2. Dr. O.K. Medical Center will not disclose any medical information to any third party (except as may be required by other health professionals) without written consent from the patient.

Cancellation Policy

Dr. O.K. Medical Center enforces a strict cancellation policy to ensure enough time for patients who need health services each day. The following rules are enforced.

  1. Dr. O.K. Medical Center requires 24 hours notice for any cancellation for any appointment. This ensures that Dr. O.K. Medical Center has enough time to fill this time-slot with another patient. A fee will be charged for all missed or late (more then 15 minutes) appointments without 24 hours notice.
  2. Cancellations must be made to a Dr. O.K. Medical Center staff member; they may not be left as messages. This means that cancellations must be made during office hours.

Patient Responsibilities

To ensure that patients receive optimal care all patients are asked to:

  1. Please be direct and honest about information and give accurate and complete medical history.
  2. Please be considerate to the doctors, nurses, therapists, support and administration staff, as well as other patients.

Financial/OHIP Policy

Dr. O.K. Medical Center’s financial and OHIP policy is designed to ensure that all services rendered are paid for at the time of service.

  1. For medical services: All patients must provide a valid (not expired) OHIP card at every visit in order to receive covered medical care.
    1. In the event of an expired OHIP card, patient will be billed directly for the service at the time of the visit. This is non-refundable.
    2. For services uninsured by OHIP, payment is required at the time of service.
  2. For Cosmetic, and Minor Surgical Procedures:
    1. Dr. O.K. Medical Center collects all payment at the time service is rendered. There are no exceptions.
    2. Dr. O.K. Medical Center provides all patients with all appropriate receipts of payment for submission to insurance companies but does not bill directly to 3rd party coverage providers.
